NURSECOORDINATOR
Cooperative Production has 31 residential homes and 5 Day Programs. Our 24/7 supervised residential programs focus on a person-centered planning approach and working with individuals to create homes adapted to meet individual needs. We provide support in life skills, social skills, and community integration, emphasizing pursuing personal goals and interests. Each day, our staff provides guidance, support, and direction as needed on new skills and tasks, behavior management, personal development, social skills, and communication. They take pride in assisting individuals in achieving goals by maintaining knowledge of their learning styles, strengths, and support needs, and ensuring that skills are taught accordingly. The Nurse Coordinator supports the health and well-being of individuals receiving residential and day services. This position monitors health needs, coordinates medical care, administers medications and treatments, maintains clinical documentation, and works closely with individuals, families, guardians, staff, and healthcare providers. Working in partnership with residential and health services leadership, the Nurse Coordinator also supports safe program operations, staff development, transitions of care, and compliance with agency and Medication Administration Program requirements. All responsibilities are carried out in a respectful, person-centered manner that protects each individual's dignity, independence, rights, and quality of life.Direct Supervisor:

- High school diploma or equivalent required; college degree preferred.
- Current Massachusetts licensure as a Licensed Practical Nurse.
- Three years of supervisory experience working with individuals with disabilities or acquired brain injuries preferred.
- Ability to successfully complete required CORI, fingerprint, and
OIG-LEIE
screenings.- Valid driver's license and an acceptable driving record.
- Ability to transport individuals using an agency vehicle and, when required, a personal vehicle for business purposes.
- Must be at least 19 years old to drive on behalf of the agency and complete all required driver training.
- Ability to physically assist individuals entering and exiting vehicles and transfer adults to and from wheelchairs or other adaptive equipment.
- Ability to provide satisfactory health documentation demonstrating the physical capacity to perform the position's responsibilities.
- Documentation of a current tuberculosis screening, renewed every two years, when assigned to a MassHealth-funded ABI or MFP program.
- Strong communication, organization, clinical judgment, and documentation skills.

ESSENTIAL DUTIES
- Monitor the general health status of individuals, including illness complaints, hygiene concerns, skin integrity, seizure activity, immunizations, dietary needs, and treatment plans.
- Assess health concerns, provide minor first aid when appropriate, and recommend or coordinate further medical attention.
- Administer medications and treatments in residential and day program settings in accordance with clinical orders and agency procedures.
- Ensure medications are properly obtained, stored, documented, and disposed of.
- Confirm that physicians' orders, prescribed diets, laboratory tests, diagnostic procedures, and treatment recommendations are implemented and completed.
- Schedule and attend medical, dental, therapy, and other healthcare appointments as needed.
- Coordinate follow-up care and communicate with physicians, pharmacists, dentists, therapists, hospitals, and other healthcare providers.
- Support hospital discharges, transitional care, and admissions into residential programs by obtaining required medical information and ensuring a safe continuity of care.
- Maintain accurate, current health records and complete all required clinical and service documentation.
- Update HCSIS and other agency systems as assigned.
- Participate in ISP, transition, discharge, and interdisciplinary team meetings.
- Communicate relevant health information to individuals, families, guardians, residential staff, and other providers in coordination with program leadership.
- Assist the Residential Coordinator with day-to-day program operations and provide appropriate feedback regarding staff performance.
- Participate in interviewing and hiring Licensed Practical Nurses.
- Assist the Director of Health Services with LPN scheduling and on-call coverage as requested.
- Facilitate staff and house meetings in partnership with the Residential Coordinator.
- Provide staff training and clinical guidance, including training required under Medication Administration Program policies.
- Monitor program compliance with Medication Administration Program requirements while recognizing that this position does not supervise medication administration by unlicensed staff under the direct authorization model.
- Ensure medical equipment, adaptive devices, and protective supports are available, clean, appropriate, and in safe working condition.
- Monitor the implementation of medical, behavioral, therapeutic, and individualized service plans.
- Complete follow-up actions resulting from internal reviews, audits, or identified compliance concerns.
- Assist with case management and service documentation responsibilities as assigned by the interdisciplinary team.
- Support individuals in accessing community, recreational, employment, and social opportunities that reflect their preferences and goals.
- Advocate for individuals' legal, civil, and human rights.
- Help build natural community supports that encourage independence and reduce reliance on paid assistance when appropriate.
- Follow all agency policies, procedures, confidentiality requirements, and professional standards.
- Attend required training and maintain current knowledge and skills relevant to the position.
- Perform other related responsibilities as assigned.
WORKING CONDITIONS
- Work assignments and schedules may change based on the needs of individuals and programs.
- The position requires travel to residential homes, day programs, medical appointments, and other community locations.
- Employees may be required to assist individuals who display aggressive or self-injurious behavior in accordance with approved behavioral support plans and agency training.
- Physical assistance may include supporting or transferring adults to and from wheelchairs, vehicles, or adaptive equipment.
- All support must be provided in a dignified manner that protects each individual's identity, self-esteem, privacy, and personal choice.
- Employees must immediately report suspected mistreatment or abuse of an individual to the Disabled Persons Protection Commission as required by law and agency policy.
