Note:
The compensation range noted above represents the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Valley Health Systems, Inc., the parent company of Holyoke Medical Center, provides a broad range of physician services in offices conveniently located throughout Western Massachusetts. Holyoke Medical Center is an independent community hospital that has served the healthcare needs of the Pioneer Valley for over 125 years. We continue to grow and address the needs of our community through quality and patient-centered care. Our Orthopedics Practice is seeking a Nurse Navigator to coordinate and oversee patient care before surgery, throughout the postoperative period, and after discharge by collaborating closely with providers, patients, and families to ensure timely, high-quality care. Overview of responsibilities include but are not limited to: Provide comprehensive patient education regarding procedures, treatment plans, and the continuum of care. Coordinate pre-procedure evaluations, testing, appointments, and clearances to support timely care. Assess patient needs and develop individualized care plans, including identifying potential risks or barriers. Serve as a liaison among patients, families, providers, and multidisciplinary healthcare teams. Monitor compliance with established care processes, protocols, and treatment plans. Identify and communicate issues that may affect the timing or effectiveness of patient care. Develop educational resources and tools to promote patient engagement and self-management. Collect, analyze, and report patient outcomes and quality data to support continuous improvement. Coordinate discharge planning, follow-up care, and post-discharge services to promote safe transitions and reduce readmissions. Monitor patient and family satisfaction, participate in quality improvement initiatives, and support the development of care management policies, procedures, and electronic tools.
Qualifications:
Education:
Graduate of an accredited school of nursing, Bachelor's degree preferred
Licenses/Registrations:
Current RN license within the
Commonwealth of Massachusetts Experience:
Three to five years of clinical related job experience, case management experience preferred
Special Aptitudes & Activities:
Strong written and verbal communication and interpersonal skills, with the ability to collaborate effectively with patients, physicians, and office staff. Demonstrates solid knowledge of practice workflow, strong organizational and prioritization abilities, and excellent attention to detail while working independently.
