Compensation Travel Pay:
Up to $2,700/week Job Summary We are seeking a dedicated Home Health Registered Nurse (RN) to provide comprehensive patient care in a home-based setting. The RN will conduct detailed patient assessments, develop individualized care plans, and ensure the delivery of high-quality, patient-centered care while collaborating with interdisciplinary teams. Key Responsibilities Perform comprehensive nursing assessments, including physical, psychological, and environmental factors Develop, implement, and revise individualized patient care plans Evaluate patient conditions and adjust care plans as needed Maintain clinical documentation that reflects skilled nursing care Monitor patient progress and report significant changes to physicians and care teams Administer medications and provide IV therapy as required Participate in interdisciplinary case conferences and care coordination Supervise Home Health Aides and review care plans regularly Educate patients and caregivers on treatment plans and health management Coordinate patient discharges and complete required documentation Ensure compliance with regulatory standards and best practices Refer patients to community resources as appropriate Required Qualifications Active Registered Nurse (RN) license in Massachusetts (non-compact state)
Minimum:
1 year of nursing experience OR 6 months of experience with a BSN CPR certification required Successful completion of pre-employment medication test Preferred Qualifications Home health or visiting nurse experience IV therapy experience Bachelor of Science in Nursing (BSN) Strong computer and documentation skills Skills & Competencies Patient assessment and care planning Case management and coordination Clinical documentation and compliance Patient and caregiver education Interdisciplinary collaboration Time management and organizational skills Additional Information 13-week contract assignment with potential extension Day shift role with occasional weekend/holiday coverage Focus on delivering high-quality care in a home health environment Performance aligned with national healthcare quality benchmarks For more details reach at or Call / Text at 516-862-1203 .
