This energetic role offers the chance to work independently while collaborating with multidisciplinary teams to deliver high-quality nursing services. As a vital link between healthcare facilities and patients, you will provide comprehensive assessments, coordinate care plans, and ensure seamless communication across care teams. Your dedication will help improve health outcomes and enhance patient satisfaction in a variety of environments, including home care, assisted living, and long-term care facilities. Responsibilities Conduct thorough patient assessments during home visits or community-based settings, including vital signs, physical examinations, and health history reviews. Educate patients and their families on disease management, medication adherence, nutrition, and health coaching tailored to individual needs. Document all patient interactions accurately using EMR (Electronic Medical Records) systems like ECaring while ensuring compliance with HIPAA regulations. Coordinate with physicians and case managers for discharge planning, follow-up care, and referrals to specialists such as cardiology or ophthalmology. Qualifications Valid RN license in good standing with current state registration. Proven experience in diverse clinical settings such as hospital medicine, emergency medicine, ICU, or long-term care environments. Strong knowledge of medical terminology, ICD coding (ICD-10), laboratory procedures, and diagnostic evaluation techniques. Familiarity with EMR/EHR systems such as Epic or Cerner for accurate medical documentation. Critical care experience preferred—especially in Level I or Level II trauma centers—and familiarity with trauma medicine protocols. Ability to perform physical examinations and assessments with attention to detail; experience working with individuals with disabilities or developmental delays is a plus. Excellent communication skills for triage calls, patient education, and interdisciplinary collaboration across outpatient clinics or home & community care settings. Knowledge of Workers' Compensation law, hospice & palliative medicine principles, pain management strategies, and infection control practices highly desirable.
