Job Description
Job Overview:
- We are seeking a compassionate and skilled On-Call Hospice Nurse to join our team. The Hospice Nurse will provide end-of-life care and support to patients and their families, ensuring comfort and dignity during their final days.
Duties:
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient vital signs and symptoms
- Provide emotional support and guidance to patients and families
- Collaborate with physicians and healthcare team members
- Coordinate care with other healthcare providers
- Educate patients and families on end-of-life care options
Skills:
- Experience in hospice, assisted living, nursing home, or hospital settings
