Full-time, part-time, or per diem options may be available About the Role Americare Behavioral Health Adult Foster Care is seeking a compassionate and skilled RN or LPN to provide nursing oversight, assessment, care planning, and member support within our Adult Foster Care program. The nurse will work collaboratively with members, caregivers, families, the interdisciplinary care team, and community providers to promote safe, person-centered care in the home environment and support each member's health, independence, dignity, and well-being. Key Responsibilities- Provide nursing oversight, assessment, care planning, and ongoing monitoring for Americare Behavioral Health Adult Foster Care members.
- Complete comprehensive member assessments, including medical history, home safety, environmental, physical, psychosocial, and functional evaluations.
- Develop, implement, and revise individualized care plans in collaboration with the interdisciplinary care team.
- Prepare and submit timely clinical documentation, progress notes, and required assessments.
- Monitor member status, identify changes in condition, and communicate promptly with supervisors, families, caregivers, and other team members.
- Educate members, caregivers, and families on care needs, medications, safety, disease management, and care plans.
- Coordinate member services and determine the need for additional professional or community-based supports.
- Provide caregiver oversight and support in accordance with Americare Behavioral Health Adult Foster Care program requirements, agency policy, and applicable state and federal regulations.
- Participate in case conferences, staff meetings, in-services, performance improvement activities, and clinical record reviews.
- Maintain professional licensure, current certifications, and compliance with agency policies and documentation standards. Qualifications
- Graduate of an approved professional nursing program.
- Current Registered Nurse or Licensed Practical Nurse license in good standing in Massachusetts.
- Minimum of one year of professional nursing experience; Adult Foster Care, community health, or home/community-based care experience preferred.
- Current CPR certification required.
- Ability to pass required criminal background checks.
- Valid driver's license, reliable transportation, and ability to travel to member homes and meetings.
- Strong assessment, documentation, communication, and care coordination skills.
- Ability to work independently in member homes while collaborating effectively with the care team.
