Description:
Wellness Registered Nurse (RN) Part-Time Contract Position The Wellness RN works with individuals and groups/residents to help them achieve their health and wellness goals and connect with peers in the elderly housing community. Assistance includes health education, supportive wellness services, and care coordination through individual encounters and group workshops on relevant health topics and chronic health conditions. Host vaccine clinics and outside speakers. Blood pressure screening. Teaching on topics such as medications management, file of life, health care proxy, advance directives, nutrition for heart health, safe exercise, mental health, and fall prevention. Home visits and monitor residents who have recently been discharged from rehab or hospital. Culturally competent in the housing/health community. The position is part-time. The RN works within the scope of practice of the MA license.
Position Details:
Part-time position: 15 hours per week
Schedule:
3 days per week, Monday, Thursdays, and Fridays Contracted 1099 position / which means that state and federal taxes will not be deducted, and the candidate will receive a 1099 form for their taxes at the end of the year The candidate must have liability insurance in place upon hire Massachusetts RN license. Additional responsibilities include teaching and support related to: Chronic disease management (including hypertension, diabetes, and heart health) Blood pressure monitoring and wellness screenings Medication management and medication safety File of Life preparation Health Care Proxy and Advance Directives Nutrition and healthy lifestyle education Safe exercise and mobility Mental health and emotional wellness Fall prevention and home safety Post-hospitalization and post-rehabilitation follow-up care Coordination with providers, caregivers, and community resources as appropriate Special wellness events may include on-site podiatry services Reliable scheduling availability for resident support and wellness services
