A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description

Job Title:

Home Health RN Assignment Length:

13 weeks with possible extension

Location:

Newton, MA Shift:

8am-4:30pm; Must be willing to work weekends. Weekend rotations are once every 6 weeks.

Pay Rate:

$75/hour

Job Summary:

As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with the Home Care's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

Experience:

1 year Certified Home Care experience - a Must!! 1 year Case Management experience- a Must!!

Job Requirements:

RN License in Massachusetts Reliable, independent means of transportation to make home visits. Fully Vaccinated against Covid-19 Who We Are?
