$45-50/hr Seeking an experienced and compassionate Registered Nurse (RN) Float - Lighthouse to provide high-quality home health care services across assigned territories. The Home Health RN will be responsible for assessing patient conditions, developing and implementing individualized care plans, and providing ongoing education and support to patients and their families. This role also includes admission processing and case management responsibilities in accordance with organizational policies, standards of community health practice, and interdisciplinary communication protocols. Accurate and timely electronic documentation is required. Required Qualifications Active RN license in Massachusetts Minimum 1 year of Certified Home Health Care experience Minimum 1 year of Case Management experience Ability to travel to patient cases as assigned Strong clinical assessment, communication, and documentation skills Important Qualification Note Please note that experience in hospice care, private duty care, or general Case Management alone does not qualify as Certified Home Health Care Management experience. Only experience gained specifically within a certified home health agency will be considered valid toward the required 1-year experience. Responsibilities Conduct patient assessments and develop individualized care plans Perform admission and case management duties Monitor patient progress and update care plans as needed Provide patient and family education regarding treatment and care Collaborate with interdisciplinary healthcare teams Complete thorough and timely electronic documentation Deliver care in accordance with organizational standards and community health practices Schedule & Additional Information Weekend rotation required once every 6 weeks Start date is dependent upon successful credentialing clearance If credentialing is not completed by the posted start date, a revised start date will be assigned (Home Health RN, Home Care Nurse, Visiting Nurse, Community Health Nurse, In-Home Nurse, Field Nurse, Private Duty Nurse, Skilled Home Care Nurse, Home Health Case Manager, RN Case Manager, Home Visit Nurse, Community-Based RN, Transitional Care Nurse, Patient Care Coordinator, Nurse Case Manager, Home Healthcare RN, Home Infusion Nurse, Hospice RN) #
