Role:
As an RN or LPN , you will be responsible for providing high-quality patient care, working closely with our interdisciplinary team to develop and implement individualized care plans, and ensuring a safe and efficient delivery of care. You will be expected to work a variety of shifts, including days, evenings, and weekends, and will be required to possess a valid MA RN or LPN license in good standing.Responsibilities:
Provide direct patient care, including administering medications, performing wound care, and conducting vital sign checks Collaborate with other healthcare professionals to develop and implement individualized care plans Maintain accurate and up-to-date patient records and documentation Participate in quality improvement initiatives and maintain a safe and clean work environment Provide education and support to patients and their families Work closely with our interdisciplinary team to ensure seamless transitions of careWhat We Offer:
