Per Diem Schedule:
Flexible Per Visit Rates Apply Responsibilitie s: Conduct admission assessments and develop personalized treatment plans. Provide routine and urgent care visits, ensuring flexibility to meet patient needs. Manage medication orders and educate patients on proper usage. Handle wound care, testing, and equipment/supply needs. Document patient progress and collaborate with the care team for optimal results. Assist in discharge planning and coordinate with community resources. Participate in team meetings and briefings to ensure cohesive patient care. Maintain patient confidentiality and report any incidents or safety concerns.
Qualifications:
Current APRN state professional license (Adult/Geriatrics specialty preferred), DEA certificate, CPR certification. Master's degree in Advanced Practice Nursing from an accredited school. At least 2 years as a Nurse Practitioner, preferably in home health care. Valid driver's license with reliable transportation. Excellent communication, problem-solving, and organizational skills. Homecare Homebase knowledge is a plus.
