The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring adherence to established healthcare standards and protocols. This role requires a strong commitment to patient-centered care, effective communication, and the ability to work independently in a home setting. Duties Perform comprehensive assessments of patients' health conditions during home visits. Develop and implement personalized care plans based on patient evaluations. Educate patients and their families about health management and treatment plans. Collaborate with other healthcare professionals to coordinate patient care and referrals. Maintain accurate and up-to-date medical records and documentation. Adhere to safety protocols and maintain confidentiality of patient information. Skills Strong knowledge of acute care practices and home health nursing standards. Proficiency in health coaching techniques to empower patients in managing their health conditions. Excellent interpersonal skills for building rapport with patients, families, and healthcare professionals. Ability to work independently while demonstrating strong organizational skills and attention to detail.
