Overview We are seeking a compassionate, highly skilled Certified Registered Nurse (RN) to join our dedicated home care team in a full-time capacity. This vital role offers an opportunity to provide exceptional, patient-centered care in a home setting, ensuring comfort, safety, and well-being for individuals with diverse medical needs. The ideal candidate will bring a strong background in acute and post-acute care, along with a genuine passion for making a positive impact on patients' lives through empathetic and professional nursing practices. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to determine individualized care plans. Manage and monitor acute and chronic conditions across various settings such as outpatient clinics, inpatient units, and post-acute care environments. Administer medications, treatments, and interventions following strict protocols while ensuring patient safety and comfort. Collaborate with physicians, case managers, and other healthcare professionals to coordinate discharge planning and ongoing care strategies. Educate patients and their families on health management, medication use, and lifestyle modifications to promote recovery and wellness. Maintain accurate medical records in compliance with HIPAA regulations and organizational standards. Perform diagnostic evaluations and assist with complex medical procedures in the home setting when necessary. Utilize critical thinking skills for patient assessment, emergency response, and utilization management to optimize care delivery. Skills Extensive critical care experience including ICU or emergency medicine environments. Experience working in Level I Trauma Centers or Level II Trauma Centers with exposure to trauma-related cases. Strong background in hospital medicine, infectious disease care, and behavioral health management. Proficiency in medical terminology, physiology knowledge, and diagnostic evaluation techniques. Skilled in patient service delivery with excellent communication skills for effective discharge planning and case management. Familiarity with HIPAA compliance, medical records management, and outpatient clinic operations. Ability to perform patient assessments accurately while managing vital signs and acute care needs. Knowledge of managed care principles, utilization management processes, and inpatient/outpatient workflows. $20.00 - $40.00 per hour
