Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review and document detailed medical, medication, and health histories Provide education on diagnoses, treatment options, and medications Identify care gaps and recommend solutions to promote optimal health Visits may include annual assessments, as well as post-discharge evaluations No treatment plan development or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Maryland Board Certification through AANP or

ANCC MSN
