A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description Spectrum Healthcare Resources has an opportunity for a Registered Nurse - Wounded Warrior Case Manager working at Joint Base Andrews in Washington, D.C.

Requirements:

Master's degree in nursing Current, full, active, and unrestricted license to practice as a Registered Nurse (Any State) A minimum of five (5) years of nursing experience. A minimum of three (3) years for ADN or two years for BSN full-time experience in clinical case management working with adults, children, families, seniors and groups. Certified Case Manager by Commission for Case Management (CCM) or American Nurses Credentialing Center (ANCC) -OR- certification eligible with 3 years of experience for ADN or 2 years' experience for BSN in full time clinical case management with children, families, seniors, or groups. BLS and ACLS Highly organized, self-directed worker able to function in a high-volume environment without distractions. Strong verbal and written communication skills. Proficient level of experience with Microsoft Office applications and strong technical aptitude.

Work Schedule:

Monday - Friday No nights and weekends 8 hours a day
