About Us:
We are a network of substance abuse clinics serving clients in the Baltimore City and County area with six locations. We are currently seeking a Registered Nurse to join us as a Health Home Care Coordinator, working collaboratively with our team to provide exceptional care for our patients.
Responsibilities:
Coordinate care for assigned participants. Ensure implementation of treatment plans in partnership with individuals and families. Conduct health promotion and education. Provide referrals and additional supports as needed.
Principal Responsibilities and Duties:
Develop wellness and prevention initiatives. Deliver health education to participants and their families/caretakers. Participate in developing initial treatment plans for Health Home enrollees. Assist in setting goals for individuals with chronic diseases or at risk for co-occurring conditions. Collaborate with community resources to address social and medical issues. Coordinate with medical providers and hospitals for admissions and discharges. Track required assessments, screenings, and immunizations. Monitor HIT tools and reports for treatment updates, medication alerts, and hospital activities.
Requirements:
Active RN license to practice in Maryland Smile often :)
