HUD VASH
Program Mental Health (MH) ed Nurse (RN) demonstrates the ability to provide mental health/medical nursing care and case management to Veterans in the community and assists Veterans in accessing mental health and medical services and VA/community resources. The

Nurse works closely with an interdisciplinary team framework to help foster independent living through appropriate management of the client's mental illness by providing assessments, planning, interventions, and evaluation of treatment. The

Nurse will preform nursing care functions and interdisciplinary collaborations within the outpatient mental health setting that include but is not limited to the following:
  • Reviews patient's history and physical assessment data and incorporates analysis into the formation of the care plan and teaching plan, including caregiver/family as appropriate. Incorporates the patient's life situations, home environment, responsibilities, and available resources into the care plan, assisting the person in coping with the current situation.
  • Collaborates with the Interdisciplinary Team to develop and coordinate treatment plans for patients requiring community health/social services. Treatment plans will reflect mutually established community health/social goals supported by community resources. Familiarizes self with community resources such as social services, transportation, etc.
  • Facilitates Healthy Living Groups once monthly in chosen locations.
  • Completes mental health, medical, and social needs assessment to refer to appropriate services within the VA continuum of care and community resources. Will develop partnerships with community agencies, especially those serving those experiencing homelessness and Substance Use Disorder (SUD) Veterans, with regular contacts and communication.
  • Collaborates/communicates regularly with the Public House Authority (PHA) case manager regarding the veterans' progress toward maintaining housing and treatment goals. Completes PHA documentation as necessary.
  • Rapidly intensifies emergencies, mobilizes resources, and assists in the management of patient crisis.
  • Demonstrates awareness of and leadership in performance improvement measures defined for the program and identifies problem areas for monitoring and evaluation. Analyzes findings and changes practice based on quality improvement findings with resultant improvement in patient care.
  • Demonstrates a thorough knowledge of psychiatric diagnoses and recognizes and reports signs/symptoms such as hallucinations, delusions, paranoia, and depression. Completes basic mental health assessment triages patients, and documents findings on progress notes, vital signs records, and other pertinent data sheets.
  • Demonstrates general knowledge of addictions (alcohol, drug, gambling, smoking disorders) and evaluates for signs/symptoms of withdrawal and coordinates the referral of patients to the appropriate treatment programs.
  • Maintains a safe work environment and demonstrates the proper use of equipment. Operates a motor vehicle and obeys all traffic laws. Maintains current license.
  • Assists with coordinating ancillary services and facilitates primary care referrals. Provides Consultation with staff to evaluate medical conditions, triage for emergent care, and contact the Primary Care/Emergency Department is indicated.
  • Performs and tracks all clinical reminder activities that include depression screenings, pain screenings, suicidal ideation screenings, alcohol and drug use screenings, TBI screenings, HIV screenings, hepatitis screenings, and other specific program-related screenings.

Qualifications
Basic Requirements:
  • English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program.
Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.
e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT)
Exception:
Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
NOTE:
Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification.
