ed Nurse MDS Coordinator

ed Nurse MDS Coordinator Hyattsville Nursing & Rehabilitation Center is seeking an experienced, organized, and detail-oriented ed Nurse (RN) MDS Coordinator to join our clinical team. If you are an RN with long-term care experience who is passionate about quality resident care, accurate clinical documentation, and regulatory compliance, we would love to hear from you! Position Summary The RN MDS Coordinator is responsible for coordinating and overseeing the Minimum Data Set (MDS) assessment process and ensuring assessments, care plans, and supporting clinical documentation are completed accurately and timely. The MDS Coordinator works closely with Nursing, Therapy, Social Services, Dietary, Activities, and other members of the interdisciplinary team to support quality resident outcomes and regulatory compliance. Responsibilities Coordinate and complete MDS assessments accurately and within required timeframes. Manage the MDS assessment schedule and ensure timely completion of required assessments. Conduct and coordinate comprehensive resident assessments. Collaborate with the interdisciplinary team to develop and update individualized resident care plans. Review clinical documentation for accuracy, completeness, and appropriate support of MDS coding. Ensure compliance with applicable federal and state regulations and facility policies. Participate in care-plan meetings and interdisciplinary clinical meetings. Identify documentation opportunities and provide guidance to nursing staff when appropriate. Assist with monitoring quality measures and resident outcomes. Maintain accurate records, reports, and supporting documentation. Participate in quality improvement and compliance initiatives. Promote a culture of quality, accountability, and resident-centered care. Qualifications Current and active ed Nurse (RN) license required Long-Term Care/Skilled Nursing Facility experience required Previous MDS experience strongly preferred Knowledge of the MDS/RAI assessment and care-planning process preferred Strong clinical assessment and documentation skills Excellent organizational and time-management skills Strong attention to detail and accuracy Excellent written and verbal communication skills Ability to work collaboratively with an interdisciplinary healthcare team Why Join Hyattsville Nursing & Rehabilitation Center?
