A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Graduate of an accredited school of professional nursing Current state Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required Must have a valid driver's license, proof of insurance and have means of transportation.
What You'll Be Responsible For:
Conducts an in-person comprehensive initial nursing assessments in the PACE clinic and in participant's homes with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conducts an in-person nursing re-assessments in the PACE clinic and in participant's homes semi annually and as needed. Participates in the nursing on-call rotation. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
