ComForCare Home Care in Baltimore, MD is hiring RN Case Managers to conduct in-home assessments and provide ongoing client care and case management. Position Overview:
We are seeking a skilled ed Nurse to provide clinical oversight, perform initial and ongoing assessments, and develop personalized care plans for clients receiving services in their homes. The RN will ensure quality care, monitor outcomes, and work closely with caregivers, families, and healthcare professionals.
Key Responsibilities:
  • Conduct comprehensive in-home assessments and reassessments
  • Create and revise individualized care plans based on clinical findings
  • Provide medication administration and skilled nursing interventions
  • Monitor and document changes in client health and status
  • Communicate regularly with clients, caregivers, and care teams
  • Initiate referrals to appropriate healthcare or community resources
  • Maintain timely, accurate documentation in accordance with regulations
  • Participate and facilitate caregiver in-service programs.
Qualifications:
  • Current Maryland ed Nurse (RN) license or mutli-state licensure.
  • CPR/First Aid Certification
  • Experience in Home Health, Hospice, and/or Geriatric Nursing preferred
  • Strong assessment and clinical decision-making skills
  • Ability to work evening and weekend shifts, as needed
  • Reliable transportation for home visits.

Compensation:
$40.00
