Responsibilities:

Provide skilled nursing care to patients in the home health setting while following established care plans. Complete thorough patient assessments and document findings, interventions, and patient responses. Communicate effectively with patients, families, and the care team to support continuity of care. Educate patients and caregivers on care needs, medications, and safety considerations as appropriate. Maintain accurate, timely documentation in accordance with assignment and facility standards.
