Role Description:

This full-time, on-site role is based in Baltimore, MD and involves conducting comprehensive nursing assessments in the home, developing and updating individualized care plans, perform supervisory visits and competency assessments and ongoing monitoring of client progress and safety. The nurse will also lead a team of caregivers through on-site training and ongoing supervision to empower caregivers and ensure quality care in the home. Qualifications Active Registered Nurse (RN) license in the state of Maryland Demonstrated clinical skills in assessment, care planning, medication management, and ongoing monitoring in a home or community-based setting. Experience or strong interest in home care, geriatric nursing, chronic disease management, and supporting aging in place. Knowledge of Medicaid waiver programs, home health regulations, and documentation standards or willingness to learn these requirements. Self-starter who thrives in a dynamic, mission-driven start up environment Strong communication and interpersonal skills, including the ability to educate clients and families and collaborate with interdisciplinary teams. Proven organizational and time-management abilities, with attention to detail and accurate record-keeping. Warm, empathetic personality is a must Ability to travel locally within the Baltimore, MD area to client homes as required; valid driver's license and reliable transportation.

$44.90 - $54.08 per hour Expected hours: 40.0 per week
