Senior Living Community / Skilled Nursing Facility Overview A senior living community is seeking an experienced RN MDS Coordinator to support resident assessment, care planning, regulatory compliance, and reimbursement processes within a long-term care setting. Position Summary / Responsibilities Coordinate the Resident Assessment Instrument (RAI) process in accordance with state and federal regulations. Complete and transmit accurate MDS assessments and supporting clinical documentation. Develop, implement, and evaluate individualized resident care plans with the interdisciplinary team. Schedule and lead interdisciplinary care plan meetings. Audit medical records to ensure documentation supports MDS coding and reimbursement requirements. Collaborate with physicians, health plans, and care teams to support insurance approvals and coverage eligibility. Serve as a resource regarding SNF prospective payment systems, quality reporting programs, and regulatory requirements. Monitor MDS-based quality measures and maintain compliance with CMS guidelines. Participate in utilization review, triple-check, QAPI, and other quality improvement activities. Assist with admissions, discharges, and resident care activities as needed. Requirements Bachelor's degree in Nursing required, or equivalent combination of education and experience. One (1) to three (3) years of long-term care MDS coordination and care planning experience required. Active Registered Nurse (RN) license required. BLS Certification required. Must obtain Resident Assessment Coordinator certification within one year of hire. Knowledge of state and federal healthcare regulations, MDS processes, and reimbursement systems. Strong communication, customer service, interpersonal, and computer skills.
