Essential Job DutiesProvide wound care consultations to new and existing participants in OADPOP or those referred from other departments and agencies.
Work with and assist new and existing participants in the OADPOP at various health centers in BCDH, including DGC, Dundalk, Eastern Family Resource Center.
Perform community outreach at Essex Library and other locations in Baltimore County as requested, promoting the OADPOP and its services. Provide blood pressure monitoring and blood glucose monitoring during field outreach.
Maintain inventory record of Naloxone at DGC; notify supervisor when an order needs to be placed. Maintain daily temperature logs of Naloxone at DGC. Send Naloxone temperature logs at the end of each month to supervisor.
Assemble Naloxone kits for distribution during Naloxone training as well as to OADPOP participants. Maintain inventory of supplies at DGC. Monitor and discard expired supplies.
Obtain improved primary medical outcomes/specialty care; reduce disease transmission; and connect to community resources that promote health stability.
Perform backpack model outreach throughout the County, including: educating and supplying participants with OADPOP services, conducting HIV and hepatitis C testing with direct linkage to care, and Naloxone training in the field.
Examples of Other DutiesPerforms other related duties as required.
