Job Description

Nursing degree, active registered nurse (RN) license and minimum 1+ years Home Health RN experience required. Applicants who do not meet these qualifications will not be considered. An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patient homes within Upper Marlboro, MD. This role focuses on providing quality, patient-centered care through comprehensive assessments, individualized care plans, and collaboration with interdisciplinary healthcare teams. Candidates should be well-organized, self-motivated, and capable of working independently while upholding clinical excellence and effective patient communication.
Key Responsibilities:
Perform comprehensive OASIS assessments and develop care plans tailored to patient needs. Administer skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management. Monitor patient progress and promptly communicate any changes to the care team and physicians. Educate patients and families on treatment plans, medications, disease processes, and preventive care. Maintain accurate, timely documentation using Home Care Home Base (HCHB). Collaborate with physical therapists, occupational therapists, social workers, and home health aides for coordinated care. Participate in case conferences, staff meetings, and continuing education activities. Ensure adherence to regulatory and agency standards for documentation and patient care.
Qualifications:
Active RN license in good standing within the state. Minimum of 1 year recent home health experience with field experience preferred. Proficiency in HCHB and OASIS assessment tools. Strong clinical assessment and critical thinking abilities. Reliable transportation, valid driver's license, and auto insurance. Excellent communication, organizational, and time management skills. Ability to work independently and manage a flexible field-based schedule.
Preferred:
BSN degree. Experience managing a caseload of 25-32 points weekly. Knowledge of Medicare and home health compliance standards.
Schedule & Contract Details:
Full-time, 13-week contract. Monday through Friday, 8:00 AM - 5:00 PM. Caseload of 3-5 patient visits per day (25-32 points weekly). Field-based position requiring daily travel within an assigned territory (mileage reimbursement provided).
