Seeking an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis within Androscoggin County, ME. This essential position involves evaluating Medicare Advantage plan members in their homes, providing valuable insights into their overall health status. Assignments may occur in various cities throughout Androscoggin County, including Auburn, Lisbon, and Poland, with potential visits to residents in Lewiston and surrounding areas.

Schedule:

Flexible work shifts are offered Monday through Sunday. Ability to self-schedule according to your availability. Minimum weekly or bi-weekly hourly commitment required.

Key Responsibilities:

Conduct comprehensive annual home health risk assessments of Medicare Advantage plan members. Review and document prior and current medical history, evaluate chronic and acute medical conditions, and assess medication usage. Educate patients on medical conditions and proper use of prescribed medications. Identify gaps in care and recommend appropriate next steps; no treatment planning or medication prescribing involved. Deliver objective, high-quality documentation for each assessment (average duration: 45 minutes per visit).

Qualifications:

Active Nurse Practitioner license in Maine. Current BLS/CPR certification. Board certification via AANP or ANCC. A minimum of six months recent NP experience preferred. Master of Science in Nursing (MSN) from an accredited nursing program.

Compensation:

Earn $80 to $90 per completed evaluation, including a stipend for both mileage and administrative time.
