Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members in patients homes Assess current and prior medical history, ongoing conditions, and medication profiles Educate patients on their medical conditions and current medications Identify gaps in care and recommend follow-up actions as appropriate Conduct visits such as annual health checkups or post-discharge assessments No requirement to create treatment plans or prescribe medications

Qualifications:

Current Nurse Practitioner license in Maine Board Certification through AANP or ANCC Minimum of 6 months of recent Nurse Practitioner experience preferred MSN from an accredited nursing program Current CPR certification (BLS)

Compensation:
