Locations can vary per shift and may include Presque Isle, Caribou, Fort Fairfield, and Washburn.

Key Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage plan members Review and evaluate both prior and current medical history, including chronic conditions and medication regimens Educate patients on their medical conditions and prescribed medications Identify gaps in care and make recommendations for follow-up or resolution No requirement to develop treatment plans or prescribe medications

Qualifications:
