Key Responsibilities:

Perform annual health risk assessments in members homes Evaluate medical history, active and prior conditions, and current medications Educate members and caregivers about management of chronic conditions and medication adherence Identify care gaps and communicate actionable recommendations Documentation and assessment onlyno treatment planning or prescribing necessary

Qualifications:

Active Nurse Practitioner license in Maine Board Certification from AANP or ANCC Current BLS/CPR certification Minimum of 6 months recent NP experience preferred MSN degree from an accredited nursing program This role offers compensation of $80-$90 per evaluation, plus mileage reimbursement and additional pay for administrative time.
