Nursing the way you were trained to do it OCFM is an independent, physician-owned Direct Primary Care practice in Hampden. We are adding a full-time RN. Our members pay us directly — a flat monthly fee — and we do not bill insurance for the care we provide. That changes the working day. No coding pressure, no productivity quota, no fifteen-minute visit template. The care we deliver here does not wait on an insurer's approval. Our physicians carry smaller panels and longer visits than an insurance-based practice can afford to, which means our nurses have room to finish a conversation instead of cutting it short. What you will do You will work alongside our physicians, medical assistant, and front desk staff. Clinical work here is shared — what follows is where you lead, not a list of things you carry alone. Triage and continuity. You are the clinical voice behind our front desk — the person a call comes to when it needs a nurse. You will build real continuity with our member families and help protect the same-day access they pay us for. Clinical care and procedures. Medication administration and immunizations, phlebotomy, EKG, point-of-care testing, wound care, and procedure support including OMT and PRP. You lead the in-office response when something goes wrong, with a physician down the hall. You oversee infection control and instrument processing, with our MA sharing the day-to-day work. Obstetrics and prenatal. Prenatal workup, fetal heart tones, fundal height, prenatal labs, patient education, and coordination with the delivering hospital. Prior OB experience is a plus; we will teach the right nurse. Care coordination. Referrals, imaging, preventive care recall, chronic care outreach, refills, and closing the loop on results — including helping members find cash-pay pricing that often beats insurance-based care. Front desk handles scheduling and records. What we need ADN or BSN from an accredited program; current Maine or compact RN license BLS required; ACLS or willingness to obtain Experience sufficient to triage independently and recognize what needs a physician now Comfort with EHR and secure patient messaging Ambulatory, urgent care, or ED background preferred. OB experience strongly preferred. Who fits here You like people — not the abstraction, the actual ones, including the difficult ones on the difficult days Warmth over the phone is a real skill, and it matters here; our members hear your voice more often than they see our faces You take satisfaction in the follow-through — the callback nobody required, the loop you closed because you noticed it was open You will advocate for a patient with a specialist's office, a pharmacy, or an insurer, and keep your composure while doing it You pitch in without keeping score; in a practice this size, everyone occasionally does something that isn't strictly theirs Few office nursing jobs include obstetrics, procedure work, and independent triage authority. If you want to keep and grow clinical skills rather than narrow them, this one is unusual. Schedule and pay Monday-Friday, daytime. No nights, no weekends, no holidays, no float pool, no mandatory overtime. The same schedule every week — you can plan a life around it, and commit to things that happen at a fixed hour. About competing offers We are not going to out-bid a hospital system, and we will not pretend otherwise. What we would ask is that you compare the whole number. A hospital rate that looks higher usually includes night, weekend, and holiday differentials — money you earn by working the hours you would rather not, on a schedule that changes month to month. Ours is what you earn on days, every week, with no shift you did not choose and no one holding you past the end of it. Run that math honestly before you decide. Then there is the part that appears on neither offer letter. Here, you will never have to tell a patient that we cannot do something because of what their insurance will approve. Ask any nurse who has had to say that how much it costs to keep saying it. Before you apply This is a small practice, and that cuts both ways. There is no nursing supervisor above you and no float pool — but there are also no layers between a question and an answer. The physician is down the hall, and decisions get made the same day. What it does ask is ownership. When something lands, you see it through rather than passing it to the next shift, and our members notice when the person who answers does not know them.
