Key Responsibilities:

Conduct home health risk assessments for Medicare Advantage plan members located in Rockland, Camden, and nearby areas in Knox County Evaluate prior and current medical history, including conditions and medication use Educate patients about their conditions and prescribed medications Identify any gaps in care and offer recommendations for improvement All visits are risk assessmentsno requirement for treatment planning or prescribing

Required Qualifications:

Active Nurse Practitioner license in Maine Board certification through AANP or ANCC Current CPR (BLS) certification MSN degree from an accredited nursing school At least 6 months of Nurse Practitioner experience preferred
