Key Responsibilities:

Conduct comprehensive, in-home health risk assessments for Medicare Advantage plan members, including annual and post-discharge evaluations. Review and evaluate prior and current medical history, existing conditions, and medication profiles. Educate members on their medical conditions and medications, empowering them to make informed health choices. Identify discrepancies and gaps in care, providing actionable recommendations for improvement. Focus solely on assessmentno requirement to devise treatment plans or prescribe medications.

Qualifications:

Current Nurse Practitioner license in Maine. Board Certified via AANP or ANCC. Current CPR certification (BLS). MSN from an accredited nursing school. At least 6 months of recent Nurse Practitioner experience preferred. Compensation for this role includes $80-$90 for each completed evaluation, in addition to a mileage stipend and compensation for administrative time. You'll be servicing Medicare Advantage members in Aroostook County, including towns such as Presque Isle, Caribou, Fort Fairfield, and Washburn, with patient assignments varying per shift.
