Delivering high-quality patient care through home health risk assessments, this per diem Nurse Practitioner opportunity offers flexibility and autonomy within Androscoggin County. In this role, you will visit Medicare Advantage plan members in their residencesincluding areas like Lewiston, Auburn, Lisbon, and Polandto conduct thorough health evaluations. The focus is on preventive assessment and empowering patients, not ongoing management or prescribing.

Key Responsibilities:

Complete annual and post-discharge home health risk assessments (approx. 45 minutes each) Review past and present medical histories, address current conditions, and reconcile medication histories Educate patients about chronic and acute conditions, as well as medication usage Identify care gaps and provide practical, actionable recommendations No responsibility for treatment plan creation or medication prescribing

Qualifications:

Active Nurse Practitioner license in Maine Board certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited nursing program At least 6 months' experience as a practicing NP is preferred Enjoy competitive compensation: $80$90 per evaluation, with additional mileage stipends and payment for associated administrative time.
