Perform annual or post-discharge home health risk assessments, averaging 45 minutes per visit Travel to residences in Presque Isle and throughout Aroostook County, including Caribou, Fort Fairfield, and Washburn Review medical history, current health status, and medication use Educate patients about health conditions and medication adherence Identify care gaps and provide recommendations (no prescribing or treatment planning required) Document findings accurately for improved care coordination

Desired Qualifications:
