Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members, each visit averaging 45 minutes Evaluate prior and current medical conditions, history, and medications Educate patients about their health conditions and medications Identify gaps in care and recommend appropriate steps for improvement No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in ME Board Certification through the AANP or

ANCC MSN

earned from an accredited nursing program Current CPR certification (BLS) At least 6 months of current NP experience preferred This per diem opportunity offers compensation of $80-$90 per completed evaluation, as well as a mileage stipend and payment for administrative work.
