Key Responsibilities:

Conduct comprehensive annual health risk assessments for Medicare Advantage members in patient homes Review prior medical history and current health conditions, as well as medication usage Educate patients on their health concerns and treatment regimens Identify gaps in care and offer appropriate recommendations No prescribing medications or creating treatment plans required Complete accurate clinical documentation following each assessment

Qualifications:

Active Nurse Practitioner license in Maine Current BLS/CPR certification Board Certified through either AANP or

ANCC MSN

from an accredited nursing school Minimum of 6 months of current NP experience (preferred) This per diem opportunity offers compensation of $80-$90 per evaluation, with a mileage stipend and paid administrative time included.
