Schedule:

Flexible shifts available all week, Monday through Sunday Self-scheduling; manage your availability around your personal commitments Choose a minimum hourly commitment each week or bi-weekly

Key Responsibilities:

Perform in-home annual health risk assessments for Medicare Advantage plan members Review patients prior/current medical history, ongoing medical conditions, and medication use Offer education regarding chronic and acute medical issues as well as medications Identify care gaps and recommend further evaluation or interventions as appropriate Documentation and reporting duties; no prescribing or treatment plan creation required

Qualifications:

Active Nurse Practitioner license in Maine Board certified as a Nurse Practitioner (AANP or ANCC) MSN from an accredited nursing program Current BLS (CPR) certification Six months or more of Nurse Practitioner experience preferred
