Each assessment typically lasts about 45 minutes and may be scheduled as either an annual health review or as a post-discharge visit.

Key Responsibilities:

Perform annual home health risk assessments for members of a Medicare Advantage health plan Review patients prior and current medical histories, along with their medication profiles and ongoing conditions Educate patients regarding their medical conditions and medication regimens Identify any existing gaps in care and make recommendations to address these gaps No prescribing of medications or development of treatment plans required

Qualifications:
