Key Responsibilities:

Perform comprehensive home health risk assessments, typically lasting about 45 minutes per visit Review and document prior and current medical history, ongoing medical conditions, and detailed medication records Offer clear education on medical conditions and medication management for plan members Identify gaps in care and suggest actionable recommendations to improve health outcomes Focus solely on assessmentno prescribing or development of treatment plans involved

Qualifications:

Active Nurse Practitioner license in the state of ME Current BLS (CPR) certification Board Certification through AANP or ANCC required Minimum of 6 months experience working as a Nurse Practitioner preferred MSN from an accredited school of nursing
