In this vital role, you will provide high-quality patient care at patient's home, ensuring safety, comfort, and optimal health outcomes. Your expertise will support a wide range of medical services, from acute care to long-term management, while fostering a positive environment for patients and the family. This position offers an exciting opportunity to make a meaningful difference in patients' lives through expert nursing practices and collaborative teamwork. Responsibilities Deliver comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management. Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems to document patient care activities thoroughly and efficiently. Assist with diagnostic evaluations such as laboratory specimen collection, phlebotomy when applicable. Support specialized care needs such as hospice & palliative medicine, dementia & Alzheimer's care, geriatrics, memory care, or behavioral health cases. Collaborate with multidisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure compliance with HIPAA and NCQA standards. Qualifications Valid RN license in good standing with current state registration. Prior experience working in home care setting is preferred but not mandatory. Familiarity with EMR/EHR systems like healthcare synergy is preferred but not mandatory. Strong knowledge of medical terminology, anatomy & physiology and infection control protocols. Experience with advanced nursing procedures including catheterization (urinary or vascular), ventilator management, tube feeding, and aseptic technique. Excellent patient assessment skills combined with compassionate communication and teamwork abilities.
