As part of our Assertive Community Treatment (ACT) team, you'll deliver recovery-focused care in real-world settings—homes, churches, community spaces—meeting people where they are and helping them move forward. Every day, you'll help individuals with mental health challenges build stability, independence, and hope. What You'll Do As an ACT Nurse, you'll serve as a vital member of a multidisciplinary team, providing holistic care to individuals with serious mental illness, emotional disturbance, or developmental disabilities.
      Your role includes:
      • Conducting nursing assessments and monitoring health needs
      • Supporting individuals' social, vocational, and community functioning
      • Delivering care in the least restrictive environments
      • Providing education, crisis intervention, and family support
      • Participating in the ACT Program's after-hours on-call rotation
      • Collaborating with internal teams and external partners
      • Supporting individuals with co-occurring disorders through IDDT model fidelity
      • Participating in the Children's Diagnostic and Treatment Team What You Bring
      • RN degree from an accredited institution with mental health coursework
      • Current Michigan RN license
      • At least 1 year of relevant mental health experience
      • Strong communication skills and functional computer literacy
      • Valid Michigan driver's license and acceptable driving record
