Provide home visitation to eligible prenatal clients and families with infants or young children who are facing challenges in health, social, or parenting concerns and can gain self-sufficiency through education, support, and referral provided by the nurse. Provide comprehensive assessments across multiple domains, including health risks, drug use, domestic violence, infant health, social work support services, case management, and psychosocial education to pregnant and postpartum women and to mothers of infants. Helps to provide intensive support services to needed clients. Provides skilled nursing care as needed to individuals and families enrolled in the MIHP program, as directed by the MD Provides care coordination, access to transportation to medical appointments, substance abuse treatment, childbirth and parenting classes. Functions as a member of a multidisciplinary team that includes a registered nurse, registered dietitian, and infant mental health specialist, managed by the MIHP Coordinator. Participates in team & chart reviews. Manages information by documenting data gathered, billing properly, and documenting services provided in program records, including the qualifying health risk assessments in accordance with policies and procedures. Data may be documented on paper or electronically. Assist maternal child health staff in development of culturally appropriate MCH materials that incorporate Native American culture and values. Provide education to parents on a wide spectrum of health, child development and parenting issues in both one on one and group settings. Promotes, administers, and documents immunizations per KBIC guidelines for participants enrolled in the Maternal Child Health programs. Assist in planning, implementation, and reporting on community education group events related to the program. Attend case management, consortia, and local meetings, as well as additional training, networking conferences, etc., per decision of the Health Promotions Manager. Provide monthly reports to the Health Promotions Manager regarding services provided. Participate in departmental staff meetings and mandatory trainings. Must be aware of community resources, including compliance with mandatory reporting laws for children at risk. Other duties as assigned by supervisor.
