Full Job Description Job Summary:
The Hospice Registered Nurse (RN) provides skilled nursing care to patients in their homes. The RN assesses patient health status, develops and implements individualized care plans, coordinates with interdisciplinary teams, and educates patients and caregivers to promote optimal health outcomes and independence. Key Responsibilities Perform comprehensive patient assessments and ongoing evaluations Develop, implement, and revise individualized care plans Administer medications, injections, and IV therapies as prescribed Provide wound care and monitor healing progress Monitor and manage chronic conditions (e.g., diabetes, heart failure, COPD) Educate patients and family members on disease management, medications, and safety Document all patient care accurately in electronic health records (EHR) Communicate changes in patient condition to physicians and care teams Coordinate care with physical therapists, occupational therapists, social workers, and other healthcare professionals Ensure compliance with agency policies, state regulations, and federal guidelines Participate in case conferences and quality improvement activities Required Qualifications Active Registered Nurse (RN) license in the state of practice Graduate of an accredited nursing program Current BLS/CPR certification Minimum of 1-2 years of clinical nursing experience (home health experience preferred) Valid driver's license and reliable transportation Ability to work independently and manage time effectively Preferred Qualifications Experience in home health, hospice, geriatrics, or medical-surgical nursing Strong wound care and IV therapy skills Knowledge of Medicare/Medicaid regulations OASIS documentation experience Skills & Competencies Strong clinical assessment skills Critical thinking and sound clinical judgment Excellent communication and patient education skills Compassion and empathy Organizational and time management skills Ability to adapt to various home environments Work Environment Patient homes in assigned geographic area Frequent local travel required Flexible scheduling may include weekends or on-call rotations
