Full Job Description Schedule:

This position would require working 3 days per week as a Care Manager and a weekend morning rotation doing phone triage, typically once every other month.

Job description:

This is a care management position, working in the office. Performing care coordination and clinical assessments of pediatric patients with chronic medical conditions, telephone triage, assists physicians with patient messages and needs, and provides patient education.
