Full Job Description Schedule:

This position would require working 3-4 days per week and a weekend morning rotation doing phone triage, typically once every other month.

Job description:

Performs care coordination and clinical assessments of pediatric patients with chronic medical conditions, telephone triage, assists physicians with patient messages and needs, and provides patient education.

Requirements:

Demonstrates leadership qualities through excellent time management, work delegation and organization skills Demonstrates creative problem solving, critical thinking skills, and excellent written and verbal communication Comprehensive knowledge of community resources Knowledge of chronic medical and mental health conditions Provides care coordination and follow-up for chronic medical and mental health conditions Ability to triage and manage complex clinical issues utilizing assessment skills and protocols. Ability to work independently as well as in a team environment Educates patients and families Advocates for patients and family members while maintaining professional boundaries Consults with members of the care team on treatment plans and follow-up care
