HCHB (Home Care Home Base/Point Click Care) (Must have experience) Overview We are seeking a dedicated PRN Hospice Registered Nurse (RN) to join our team in Wayne County. This role offers flexible scheduling to support our hospice patients by delivering compassionate, expert nursing care. The ideal candidate will possess extensive clinical experience, including critical care, trauma, and post-acute settings, with a strong focus on patient-centered care and interdisciplinary collaboration. Responsibilities Provide comprehensive nursing assessments and develop individualized care plans for hospice patients. Administer medications, manage symptoms, and deliver end-of-life comfort measures in accordance with best practices. Coordinate with interdisciplinary teams to ensure seamless patient care and discharge planning. Monitor vital signs, evaluate patient responses to treatments, and adjust care plans as necessary. Educate patients and families about disease processes, medication management, and advance directives. Document all patient interactions accurately within medical records while maintaining HIPAA compliance. Participate in case management activities, including intake assessments and utilization management to optimize patient outcomes. Experience Candidates should have prior experience in hospital or inpatient settings such as ICU, medical-surgical units, or emergency rooms. Critical care experience, including ICU or Level II trauma center backgrounds, is highly desirable. Additional preferred qualifications include familiarity with post-acute care environments such as outpatient clinics or home health services; experience with infectious disease management; knowledge of physiology; and skills in discharge planning and diagnostic evaluation. Strong understanding of medical terminology, patient assessment techniques, and managed care principles is essential. Experience working with diverse populations—including geriatrics, pediatrics, behavioral health patients—and proficiency in medical records documentation are highly valued. Candidates must demonstrate excellent patient service skills and a commitment to compassionate end-of-life care.
