This hybrid role is based in Canton, MI , with flexibility for remote work. We are looking for an RN who is fluent in Spanish and passionate about helping patients navigate their healthcare needs. In this role, you will guide patients through the healthcare system, coordinate care, proactively address potential medical concerns, and help ensure patients receive the appropriate treatment and support. You'll play an important role in improving patient outcomes while building meaningful relationships with the patients and families you serve. Key Responsibilities Communicate with patients and families in Spanish throughout the case management process Navigate and coordinate care across healthcare systems Identify and address potential medical concerns Develop and implement individualized care plans Coordinate appointments, referrals, follow-up care, and services Support patients with chronic and complex conditions Provide patient education and help patients understand their treatment plans Collaborate with providers, families, and interdisciplinary care teams Maintain accurate documentation and ensure compliance with healthcare regulations Manage an assigned caseload while staying organized and responsive to patient needs Registered Nurse (RN) Requirements Active and unrestricted RN license in the state of Michigan Fluent in Spanish Minimum of 1-2 years of clinical experience Case management, home health, rehab, or acute care experience preferred Strong understanding of care coordination, discharge planning, and acute/chronic disease management Knowledge of healthcare regulations, documentation standards, and compliance requirements Experience with EMR/EHR systems Excellent clinical judgment and critical thinking skills Strong communication and patient education abilities Ability to work independently, prioritize tasks, and manage a flexible schedule Valid driver's license and reliable transportation What We Offer Competitive pay + benefits after 90 days Fully paid training•we invest in your success Hybrid work model•approximately 50/50 in-office and remote Full-time opportunities Structured ramp-up period to build your caseload Generous compensation per case management activity
