Job Description

Job Overview We are seeking a dedicated and compassionate Registered Nurse specializing in Long Term Care to join our healthcare team. In this vital role, you will provide comprehensive nursing care to residents in a long-term care setting, ensuring their health, safety, and well-being are prioritized. The ideal candidate will possess extensive clinical expertise, excellent patient assessment skills, and a commitment to delivering high-quality, person-centered care. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing support and medical attention. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health histories, to develop individualized care plans. Administer medications and injections accurately, including IV infusions and medication management in accordance with physician orders. Monitor residents' conditions closely, recognizing signs of acute pain or deterioration, and respond promptly with appropriate interventions. Manage complex medical procedures such as wound suturing, catheterization, ostomy care, and ventilator management. Coordinate with interdisciplinary teams to facilitate discharge planning, case management, and compliance with regulatory standards such as NCQA and HIPAA. Maintain detailed and accurate medical documentation using electronic health record (EHR) systems like Cerner or Athenahealth. Provide specialized care for diverse populations including memory care, behavioral health patients, individuals with developmental disabilities, and those requiring hospice or palliative medicine. Ensure infection control protocols are strictly followed to prevent infectious disease transmission within the facility. Participate in ongoing training related to trauma medicine, ICU procedures, emergency response (Level I & II trauma centers), and critical care transport. Support residents' nutritional needs through tube feeding management and collaborate on pain management strategies for acute or chronic conditions.

$55.00 - $58.00 per hour
