This is a Pay Per Visit (PPV) position offering flexibility, autonomy, and the opportunity to provide one-on-one patient care in the comfort of patients' homes.
Schedule:
  • Monday-Friday
  • 8:00 AM
  • 5:00 PM
  • 1 weekend per month
  • 2 holidays per year

  • Active RN License in Michigan
  • Associate Degree in Nursing or higher
  • 1+ year RN experience
  • BLS Certification
  • Valid Driver's License
  • Reliable Transportation & Auto Insurance Home Health experience preferred but not required. Responsibilities
  • Conduct patient assessments and develop individualized care plans
  • Administer medications and treatments as ordered
  • Monitor patient progress and document care accurately
  • Educate patients and families on treatment plans and self-care
  • Coordinate care with physicians and interdisciplinary healthcare teams
  • Maintain accurate electronic medical records (EMR)
  • Travel locally to patient homes within assigned territory
  • Ensure compliance with HIPAA and nursing best practices Qualifications
  • Strong clinical assessment and communication skills
  • Ability to work independently and manage time effectively
  • Experience with EMR systems preferred
  • Previous Home Health, Hospice, Med-Surg, ICU, or Skilled Nursing experience is a plus
  • Compassionate and patient-focused approach to care
