Part-time Schedule:
26 hours per week 12-9 PM, 3 days/wk. and every other weekend. Our mission is to ensure quality of life, comfort, and peace for our patients and their loved ones, regardless of age, diagnosis, or financial ability. Job Summary We are seeking a dedicated and detail-oriented Registered Nurse, Admissions, to join our compassionate team. The ideal candidate will possess extensive clinical expertise in acute and post-acute care settings, with a focus on patient assessment, intake procedures, and discharge planning. This role involves coordinating admissions processes efficiently while delivering exceptional patient service within a dynamic healthcare environment. Duties Conduct comprehensive patient assessments upon admission, including vital signs, medical history review, and physiological evaluation to determine appropriate care plans. Manage the intake process efficiently by collecting accurate medical records and ensuring compliance with HIPAA regulations to protect patient confidentiality. Collaborate with interdisciplinary teams to develop individualized care plans tailored to each patient's needs, including those requiring critical care or specialized services such as ICU or PICU. Facilitate discharge planning by coordinating post-acute care services, outpatient follow-up appointments, and ensuring seamless transitions for patients. Maintain detailed documentation within electronic medical records systems while adhering to hospital policies and medical terminology standards. Participate in utilization management and diagnostic evaluation processes to optimize patient outcomes and resource utilization. Provide education and support to patients and families regarding their care plans, hospital procedures, and relevant health information. Qualifications Valid Registered Nurse (RN) license with current registration in the state of practice. Proven experience in hospital medicine, emergency medicine, ICU, or Level I/II trauma centers; experience in infectious disease care is highly desirable. Strong knowledge of physiology, medical terminology, acute care protocols, and discharge planning procedures. Prior experience with inpatient or outpatient settings including pediatrics (PICU), geriatrics, behavioral health, or medical-surgical units. Familiarity with HIPAA regulations, medical records management, case management practices, and utilization management processes. Excellent patient assessment skills coupled with the ability to manage complex cases involving diagnostic evaluation and post-acute care coordination. Ability to thrive in fast-paced environments requiring critical thinking, effective communication, and teamwork.
